Be in case management with ASA or a referring agency
Residence
Live in the Travis, Williamson, Caldwell, Hays or Bastrop counties
Meet Income Requirements
150% FPL and below
Eligibility
Maintain eligibility documents with Food & Nutrition staff every six months
Medical Nutrition Therapy
Nutrition Counseling & Group Nutrition Education for people living with HIV

Meet Eligibility
Be a Food Bank client and meet all eligibility requirements
Nutrition Health Risk Screening
Complete a Nutrition Health Risk Screen with your Case Manager.
Entry into program will be based on screening and client needs
Meet with ASA Dietitian
Complete initial assessment with our Dietitian. Together you will set nutrition goals and a care plan.
Regularly Meet with Dietitian
Meet with Dietitian during Food Bank distribution hours. If Client is unable to meet during these hours, an appointment can be scheduled with our Dietitian.
Commit to our Program
Program Commitment is a minimum of three months
